I had dragged the "Read Status" column so that it was third in position from the left, after "Creator" and "Year." "Read Status Column Format" was set to "Show only icon" with the extension icon as column header. After changing one of the custom reading status names from "To Read" to "Must Read," the Read Status column reset by moving to the 4th (rightmost) position and the "Show only icon" setting was no longer heeded.

I can reproduce the position of the Read Status column changing when it's refreshed (when you change any of the read status names, or formatting). I think this is just a bug with custom columns in Zotero, but I'll have a check to see if there's any workaround. Just to confirm, it doesn't change position when you restart Zotero does it?
I can't reproduce the "show only icon" setting being reset when the read status column is updated though. Does the same thing happen for you when you perform the same set of steps again? If so, do you see anything in the Zotero error console (Tools > Developer > Error Console)? Are you using the latest version of the extension?
@Dominic-DallOsto I also suffer from this issue. This may be not bug in Zotero. I also use the Zotero Style, which does not exist such problem. The position of the column "Read Status" of your plugin always change after restarting the Zotero.
